Privacy Policy
Kawakami Clinic, Medical Corporation Ichiguukai (“the clinic”), regards the proper handling of patients’ personal information as an important responsibility of a medical institution. We handle personal information as described below, in accordance with Japan’s Act on the Protection of Personal Information and the Ministry of Health, Labour and Welfare’s guidance for medical and care providers.
1. Purposes of use
The clinic uses patients’ personal information for the following purposes.
To provide medical care
- Providing your medical care at the clinic, including examinations, tests and treatment
- Coordinating with other hospitals, clinics, pharmacies, home-visit nursing stations and care providers
- Responding to inquiries from other medical institutions
- Asking outside doctors for their opinion or advice on your care
- Outsourcing work such as laboratory testing
- Explaining your condition to your family or others close to you
For billing and administration
- Payment and health insurance administration at the clinic
- Submitting insurance claims to the review and payment organizations
- Responding to inquiries from review and payment organizations and insurers
- Claims to, and inquiries from, government agencies about publicly funded medical care
For running the clinic
- Accounting, and reporting of medical incidents
- Improving our services and maintaining and improving our operations
- Case reviews within the clinic to improve the quality of care
Other purposes
- Reporting health checkup results to the company or organization that commissioned the checkup
- Consulting with or reporting to professional bodies and insurers about medical liability insurance
- Providing information to external auditors
- Notifications to government agencies required by law
My Number Card as health insurance card
The clinic uses online eligibility confirmation. If you use your My Number Card as your health insurance card, we obtain your past medication and health checkup information with your consent and use it in your care.
If you do not agree to any of the sharing with other medical institutions described above, please tell the reception desk. If you do not tell us, we will assume you agree. You may withdraw or change your request at any time.
2. Sharing with third parties
Except in the cases below, the clinic does not provide personal information to third parties without your consent.
- Sharing within the purposes in section 1, such as coordinating with other medical institutions (unless you have told us you do not agree)
- When required by law
- When necessary to protect a person’s life, body or property and it is difficult to obtain your consent
- When especially necessary to improve public health and it is difficult to obtain your consent
- When we need to cooperate with a national or local government body carrying out duties set by law
3. Outsourcing
The clinic may outsource some work, such as laboratory testing, part of health insurance administration, and the operation and maintenance of information systems such as electronic medical records. We choose contractors that handle personal information properly and supervise them through contracts and other means.
4. Security measures
To prevent leaks, loss or damage of personal information, the clinic:
- has appointed a person responsible for personal information and set rules for handling it;
- requires staff to keep information confidential and trains them in handling personal information;
- controls access to areas where medical records are kept and guards documents and equipment against theft or loss;
- restricts access to electronic medical records and other systems and protects them against unauthorized access.
5. Requests for disclosure
You (or your legal representative) may ask the clinic to disclose, correct, add to, delete, stop using, or stop sharing the personal information we hold about you, including your medical records. Please ask at the reception desk. We will confirm your identity and respond in accordance with the law.
- A fee may be charged for disclosure, such as providing copies of medical records.
- In some cases set by law we may be unable to meet your request. If so, we will explain why.

- This site has no contact forms or user registration and does not collect your name or contact details.
- We do not use analytics tools (such as Google Analytics) or advertising cookies.
- Your “Larger text” setting is saved only in your own browser and is not sent to the clinic.
- This site is delivered through Cloudflare. For secure delivery, Cloudflare may process connection records such as your IP address.
External services
- Online booking uses an external booking service (DigiKar Smart). The information you enter is used for your appointment at the clinic and is managed under that service operator’s terms.
- The map on the Access page is provided by Google Maps. Google may collect information about your visit when the map is displayed (Google Privacy Policy).
